An abortion is unsafe when it is carried out either by a person lacking the necessary skills or in an environment that does not conform to minimal medical standards, or both. The moral obligation is dictated by the magnitude of the health problems caused by unsafe abortion (in terms of prevalence, mortality, and morbidity), the health inequities and social injustices that poor women experience when they cannot access safe abortion, the many women who have called for action, and an international consensus—including (p. 404) statements and guidelines from the WHO—that recognizes unsafe abortion as a global public health problem. The availability of affordable Women who have unsafe abortions are at risk of serious medical problems, including incomplete abortion, hemorrhage (heavy bleeding), infection, uterine perforation (caused when the uterus is pierced by a sharp object), and damage to the genital tract and internal organs (by inserting dangerous objects such as sticks, knitting needles, or broken glass into the vagina or anus). Abortion law in Zambia allows a woman to seek the termination of pregnancy when her own life and health, or the health of other members of her family, may be put at risk by the pregnancy, or when the fetus may be expected to be damaged or diseased. Even so, many illegal abortions are performed each year in Zambia. Thus, the initial increase in the number of legal abortions was likely due to the declining demand for illegal abortion services as legal abortion became available.

Laws against abortions thus do not reduce abortions, but they do cause another very serious problem (Barot, 2011). When abortion is illegal, women either must have an abortion from an unskilled provider in unhygienic conditions, or they may try to induce their own abortion by inserting an object into their uterus or by ingesting a toxic substance.
